During surgery, hair follicles are obtained from a portion of the scalp where there's a full growth of hair. A handful of these follicles are transplanted into parts where hair is scarce or non-existent. With hair transplantation among women, thicker hair (which is usually at the back of the scalp) is redistributed to different portions of the head where thinning occurs. Larger grafts behind the hairline are generally needed to produce more density in female patients.

Generally, transplanted hair may take a considerable amount of time to regrow and may not look as it initially did before hair loss occurred. This is often the case with hair replacement for men. However, because of the generally longer length of female hair, the scalp on a female patient will be concealed in natural hair within just a few months following surgery.
